New Patient Forms

New Patient Information

Each new patient will need to fill out these forms when coming into our treatment centers. To save yourself a significant amount of time at your first visit, we encourage you to fill these forms out ahead of time. To do so, please download the forms to your computer and print them out at home. If you have questions or concerns while filling out any of these forms, fill out the form to the best of your ability. When you come in for your visit, please bring that form with you to our office, where our staff will be more than willing to assist you with those questions.

What To Expect and Bring on Your First Visit

• Please check with your insurance provider to see if they require a referral. If you are unsure about this, our office can assist with this process.
• Schedule your appointment by calling our office at (480) 646-8440.
• For your first visit to our office, we request that our patients plan to arrive 30 minutes prior to the appointment time.
• In order for our office and staff to provide you with the best, most efficient care, please have the following items with you for your first visit:
  • Photo ID
  • Insurance Card
  • Insurance Referral (If applicable)
  • A list of your current medications and dosages
  • Information about other treatments or medications you may have tried in the past
  • Medical records that may be relevant (Radiology Tests, Pathology Reports, Labs, etc.)
  • Your preferred pharmacy information
